Front of Ornament

This part of the form is for your picture or image.  

This form may be used if you are ordering a single ornament or multiple ornaments with the same text and image.  When ordering different ornaments, each ornament with different images or text will need a new form filled out.

JPG, JPEG, and PNG files supported. Uncropped photos are best. Image resolution must be at least 150 DPI.

You may upload a maximum of two images. The best one will be chosen for your image on the front.

By providing me with your image(s), you are giving me permission to use your image(s) to create and personalize the ornament you are purchasing. Your image will NOT be shared or made public. Once your order is complete and delivered, the image(s) you uploaded will be deleted.
