Custom Order Form for Personalized Ornaments.

This form must be filled out so that I may process your ornament order. Please complete thoroughly and to the best of your ability.

This part of the form is for the text that you're requesting for the back of the ornament.

Place a check mark below for each field you are requesting. You can choose one or all three.

If you want a surname on the back of the ornament, type it in the field above. I will use it exactly as entered. Examples: The Smith Family, The Smiths, Mary Smith, or The Smith's Home.

This part of the form is for your picture or image.  

This form may be used if you are ordering a single ornament or multiple ornaments with the same text and image.  When ordering different ornaments, each ornament with different images or text will need a new form filled out.

You may upload a maximum of two images. The best one will be chosen for your image on the front.

JPG, JPEG, and PNG files are supported. Image resolution must be at least 150 DPI.  Recommended minimum size is 1200 x 1200 DPI. The maximum upload size is 20 MB (megabytes). Nude, pornographic, or vulgar images will not be accepted for production.

By uploading your image(s), you warrant and represent that you own or otherwise control all the rights to your image(s), including all the rights necessary for you to provide, post, upload, input or submit the image(s). By providing me with your image(s), you are giving me permission to use your image(s) to create and personalize the ornament you are purchasing. Your image will NOT be shared or made public. Once your order is complete and delivered, the image(s) you uploaded will be deleted.
